I hereby authorise Gemini and Kiera to perform upon myself a beauty/medical enhancement. If any unforeseen conditions arise during the course of the procedure(s) I further request and authorise her to use her full judgement and do whatever she deems advisable and necessary in the circumstances.

I understand that Gemini or Kiera has the right to refuse any treatment to any patient for any reason.

COVID-19 Vaccine and Facial Fillers:

Following COVID-19 vaccinations, inflammatory side effects like redness and swelling have been found in some patients at sites of filler injections.

The side effects are rare and easily managed with antihistamines, corticosteroids, or injections of hyaluronidase.

Sensitivity/patch tests:

I understand that a patch test is required up to 24 hours before your appointment, if you do not have a patch test then you accept full responsibility and can not/will not hold Gemini’s Beauty or any therapist at Gemini’s Beauty accountable for any reactions/allergies you have prior to your treatment. If I get a reaction, a free removal is offered but no refunds will be granted. It is your responsibility to arrange a patch test, we are open Monday-Saturday 10am-4pm.

I understand that a sensitivity test for eyelash extensions, any form of facial hair/skin tinting, pigment does not guarantee that I will not have an allergic response. I am aware that allergic reactions to eyelash extension glue, any form of facial hair/skin tinting and pigment are rare and therefore accept all responsibility if an allergic response does occur.

I am aware that a sensitivity reaction to anaesthetics can occur and therefore accept all responsibility if a sensitivity reaction does occur.

General terms & conditions:

I accept and understand that Gemini’s Beauty meets the highest hygiene standards, and that sterile disposable needles are used for each individual client, procedure and visit.

I understand and accept that each procedure is a process requiring multiple applications of dermal filler to achieve desirable results, and that 100% success cannot be guaranteed.

I understand that a cosmetic/medical enhancement is an invasive procedure and that the infusion process can therefore be uncomfortable.

I am aware that the result of the procedure is determined by the following factors:

  • Medication that I am currently taking and/or have taken in the past.
  • Skin characteristics – i.e. dry/oily/sun-damaged.
  • Natural skin undertones.
  • Alcohol intake and smoking habits.
  • General stress.
  • A compromised and/or weak immune system.
  • A poor diet.
  • Post procedure care treatment.

I have been advised that upon completion of the procedure there may be swelling and/or redness of the skin, which should subside within 1-30 days dependant on lifestyle habits and correct aftercare. In some cases, bruising can occur. I have been advised that I can resume normal activities immediately following the procedure. However, using cosmetic materials, prolonged exposure to water, excessive perspiration and exposure to the sun should be limited for up to two weeks following the infusion process.

I agree to inform my doctor of my permanent cosmetic enhancement if I require an MRI scan within three months after undergoing any cosmetic treatment.

To my knowledge I do not have any physical, mental or medical impairment and/or disability that may affect my well-being as a direct or indirect result of my decision to have a cosmetic procedure.

I confirm that I am not under the influence of drugs or alcohol.

I understand that if I do not allow the practitioner to complete the procedure, I accept all responsibility for the impending result.

For the purpose of documentation, I consent to the taking of “before” and “after” photographs for said procedure(s).

I understand that this treatment uses dermal filler which consists of hyaluronic acid that is injected into the skin in order to correct facial lines, wrinkles and folds, enhance the shape and volume of a client’s lips and/or facial contours.

I accept that the dermal filler used by Gemini’s Beauty conforms to the current safety standards in the UK. I understand that it is a non-permanent procedure and that a repeat procedure may be required to maintain the desired results.

I declare that the methods of use and indications for this treatment/product have been explained to me by my practitioner and that I have had the opportunity to have any questions answered to my satisfaction.

I fully understand that, in the case of an injectable procedure, complications such as bruising, swelling, hematomas, vascular occlusions and other issues can occur. If any of these issues were to arise, I hereby state that my practitioner from Gemini’s Beauty will not be liable for such issues. I also state that I will release these said companies and practitioner from any claims, law suits etc.

I declare that I have been specifically informed of the following:

  • That after the injection, some common reactions may occur. These include mild swelling, redness, pain, itching, discolouration and tenderness of the site. Most of these issues typically resolve themselves within 1-2 days after an injection into the skin and within a week after an injection into the lips. I declare that if any of these issues persist for longer than expected I will contact my technician and GP directly and immediately.
  • That other types of reactions are very rare. I do, however, acknowledge that a small number of people are susceptible to localised reactions which are thought to be of a hypersensitive nature. Such reactions usually consist of swelling at the implant site which sometimes affect the surrounding tissues. Redness, tenderness and acne-like formations have also been reported as rare side effects of dermal filler injections. These reactions have previously either started a few days after the injection or after a delay of 2-4 weeks. The reactions have been described as mild to moderate and self-limiting. I agree that I have been made aware of these rare reactions and that I will contact Gemini/Gemini’s Beauty immediately if I experience any of these symptoms.

I absolutely understand that micropigmentation/ injectable aesthetics is an art process that is not an exact science. I further understand that every client heals differently. I understand that this is an elective procedure and is not medically necessary. If heavy make-up is required then I accept that I may require additional work, which I understand is chargeable.

I agree to contact the technician before any third parties if I have any queries or concerns regarding the procedure.

I agree that each treatment will be charged for individually and according to the amount of material used.

I understand that all prices are subject to change without prior notice.

I agree to strictly adhere to the typed after-care instructions posted/handed to me, and only use the after-care products given and/or recommend to me by the practitioner.

I understand that complications are possible, particularly if post-procedure aftercare instructions are not followed correctly. I confirm that if I get an infection post-procedure, I will visit my Doctor immediately and fully accept that it could be due to the fact that I do not live in a sterile environment.

Booking & fee policy:

A non-refundable 50% booking fee will be taken for all future appointments. This can be payable by dropping off cash to Gemini’s Beauty Charlwood address or by instant bank transfer. Your deposit will be taken off the treatment price.

I understand Gemini’s Beauty has a strict No Refunds Policy due to simple fact of being ‘self-employed’. This means that under no circumstance will a refund be granted and by booking any appointments I take full responsibility and accept this.

By booking your appointment(s), you are agreeing to all of these Terms & Conditions. You are accepting responsibility and will not hold Gemini’s Beauty accountable for any allergies/reactions/harm caused prior to any treatment performed at Gemini’s Beauty.

I confirm that when booking any beauty treatments I am over the age of 16 years old.

I confirm that when booking any cosmetic/aesthetic treatments I am over the age of 18 years old.

Cancellation policy:

Failure to attend a scheduled appointment and failure to give 48 hours notice of a cancellation or rearrangement of an appointment will waive your rights to using your booking fee towards a different appointment. In addition, Gemini’s Beauty reserves the right to charge 50% of the procedure as a cancellation fee.

I confirm I have read the Terms and Conditions of Gemini’s Beauty at the checkout before paying my booking fee.

I acknowledge that paying a deposit/booking fee to Gemini’s Beauty confirms that I have read and agree to the Terms and Conditions of the company.

A cancellation period of 48 hours prior to your appointment is required, if I cancel less than 48 hours or ‘no show’ I will pay the full appointment price before booking another appointment.

If I cancel after the 48 hour period but rescheduling my appointment, a new booking fee will be required.

If I fail to confirm my appointment when I receive a confirmation text then I understand that I risk loosing my appointment and my booking fee.

If I am over 15 minutes late to any appointment then I understand that my appointment risks being void and the full amount will still need to be paid by myself.

Privacy policy:

The collection, processing, storage and protection of any personal data you provide is of key importance to us and it will be treated securely in accordance with the Data Protection Act 1998.

All information given to us by you will be used only in accordance with EU GDPR regulations, and if, at any time, you wish to be removed from any of our mailing lists or records, you should write to us via email on geminisbeauty@outlook.com.

In the unlikely event that you receive faulty products, please notify us within 14 days of receipt of your order. We will replace, refund or exchange any products that are deemed faulty at no cost to you. If you wish to return any products for fault-related reasons, the products must be sent back to our head office for testing before any replacement, refund or exchange takes place. If the products are deemed faulty, we will also refund the shipping costs to you. When making an order we do not require you to send us your certification before completion. When you make an order, this constitutes that you are confirming to us that you are qualified and that LashBase hold no liability for incorrect use due to not being professionally trained for the products in question.

Training course(s) terms and conditions:

Cancellation Policy: You can cancel your training course and receive a full refund by giving at least 14 days notice. If you cancel your course without giving 14 days prior notice we will be unable to refund your deposit/booking fee.

Late arrival: Please let us know as soon as possible if you are running late. We can delay the start for up to 5 minutes. After that time the course will start without you.

No show: If you do not turn up on the day of your course you will not be given/granted a refund.

Rescheduling: If you would like to change the date of your course, please let us know as soon as possible and we will attempt to get you onto an alternative course.

Cancellation by Gemini’s Beauty: Gemini’s Beauty holds the right to reschedule or cancel a course date if there are insufficient numbers booked or if circumstances arise that are out of our control. Gemini’s Beauty will work with you to reschedule another course date. If no dates are suitable, Gemini’s Beauty will offer a full refund of any money paid towards the course. Gemini’s Beauty will offer no further compensation.

Refunds: All training courses are non-refundable, unless cancelled before 14 days or cancelled by Gemini’s Beauty. You purchase each course knowing that no refunds are granted.
